Dr. Dyanna Haley-Rezac, PT, DPT, OCS, CSCS, CKTP,
Dyanna graduated Cum Laude from West Virginia University with a Bachelor of Science in Exercise Physiology in 1998 and from Slippery Rock University in Pennsylvania in 2001 with her Doctor of Physical Therapy. In 2007, Dyanna received her Board Certification as an Orthopedic Specialist from the American Board of Physical Therapy Specialties. She is also a Certified Strength and Conditioning Specialist and Certified Kinesio Taping Practitioner. Dyanna is a co-owner of Rezac & Associates Physical Therapy, LLC in Colorado Springs, CO , is an Affiliate Faculty member at Regis University Doctor of Physical Therapy Program and is a guest lecturer and Advisory Board Member for Pima Medical Institute Physical Therapist Assistant Program in Denver. She is a member of the American Physical Therapy, National Strength and Conditioning Association and American Academy of Orthopedic Manual Physical Therapists.
Dr. Scott Rezac, PT, DPT, OCS, CSCS, CKTP, CEAS,
Scott graduated with his Bachelor of Science in Kinesiology and Health Promotion from California State Fullerton in 1998 and his Doctor of Physical Therapy in 2001 from University of Southern California. He received his Board Certification as an Orthopedic Specialist from the American Board of Physical Therapy Specialties in 2006. He is also a Certified Strength and Conditioning Specialist, Certified Kinesio Taping Practitioner and a Certified Ergonomics Assessment Specialist. Scott is a co-owner of Rezac & Associates Physical Therapy, LLC in Colorado Springs, CO and is a guest lecturer and Advisory Board Member for Pima Medical Institute Physical Therapist Assistant Program in Denver. He is a member of the American Physical Therapy, National Strength and Conditioning Association and American Academy of Orthopedic Manual Physical Therapists.
Credits
The following boards have approved this course – Participants will receive a certificate at the completion of the seminar for 9.6 contact hours (NY licensees), 8 credit hours (PA licensees), 8 hours (NJ licensees), and 8 hours (SC licensees). Columbia University is approved by the New York State Education Department as a provider of continuing education for physical therapists and physical therapist assistants. This seminar has been recently approved in Colorado, Pennsylvania, New Jersey and South Carolina and the North Carolina Physical Therapy Board recognizes seminars approved by other state boards. The North Carolina Occupational Therapy board approves this seminar for 3 contact hours.

Requests for refunds made before July 23, 2010 will be subject to a $35 administration fee. Requests for refunds made on or after July 23, 2010 are non-refundable. Payments made for group discounts are nonrefundable but will gladly accept replacement participants. Healthcare Professions Seminars, LLC reserves the right to cancel a seminar and will refund in full the registration fee only. HPS is not responsible for non-refundable airfare.
